WHAT'S NEW IN FINALFORMS?

2019-2020 SCHOOL YEAR
For Parents completing the electronic pre-participation physical evaluation (E-PPE) for the first time during the 2019-20 school year, the questions will not change. The only change will be noticeable when the parent prints the PDF file. The new 'Physical Examination Form' and 'Medical Eligibility Form' will be included in the printable PDF file.

2020-2021 SCHOOL YEAR
For parents completing the electronic pre-participation physical evaluation (E-PPE) for the 2020-21 school year, all updated questions and documents will be included.

The latest version, which has been authorized by the OHSAA and IHSAA was developed for use with youth and adolescent student-athletes by the American Academy of Pediatrics in its 5th edition of the monograph, PPE, Pre-Participation Examination. The AAP was advised by a working group representing the American Academy of Family Physicians, the American College of Sports Medicine, the American Medical Society for Sports Medicine, the American Orthopedic Society for Sports Medicine and the American Osteopathic Academy of Sports Medicine.
